Job Description Job Description: Nurse Practitioner, Primary Care The Family Practice Nurse Practitioner, specializing in primary care, will work closely with the Medical Director (MD) to provide excellent clinical direction to HM Wellness Clinics. They will direct, implement, and provide a plan of care for patients at HM Wellness clinics. They will provide general preventative care, conduct check-ups, treat both acute and subacute illnesses, order lab tests, and prescribe medication for patients. The Family Practice Nurse Practitioner must have the ability to travel to all clinic sites, currently located in DFW and surrounding areas Monday-Friday, 1 to 2 days per week. This will be based on a rotating schedule to be developed by the Area Administrator and based on patient needs. Join the At Your Door Labs family today www.atyourdoorlabs.com Key Responsibilities: Performs physical exams and patient observations Records patients' medical histories and symptoms Creating patient care plans and contributing to existing ones Ordering, administering, and analyzing diagnostic tests Monitoring and operating medical equipment Diagnosing health issues and plan of care for patients Administering medicine and other treatments Detecting changes in patients' health and modifying treatment plans, as needed Consulting with healthcare professionals and families Training patients and their families to manage and prevent illness and injury Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities Facilitates the patient---s transition within and between health care settings, e.g. admitting, transferring, and discharging patients Facilitates staff, patient and family decision making by providing educational tools Monitor and provider start-up status of clinics Provide coverage to additional clinic locations, as needed and assigned All other duties as assigned Required Skills and Abilities: Excellent people management skills Excellent written and verbal communication skills Ability to multi-task and meet deadlines Proficient computer skills Excellent organizational and time management skills Strong attention to detail Strong character and integrity Solutions focused with great problem-solving skills Maintain high-level of professionalism and employee confidence Ability to work independently, making sound and appropriate decisions and escalating when appropriate Education & Experience: A minimum of 2 years--- experience in Nurse Practitioner role Graduate of an accredited school of nursing Graduate of an accredited Nurse Practitioner Program Current Registered Nurse Licensure in State of Texas Current APRN/Nurse Practitioner Licensure in State of Texas Current Board Certification in adult primary care or family practice specialty area Current Medicare provider enrollment with active PTAN number Current TX Medicaid provider enrollment with active TPI number Current provider card in Basic Life Support (BLS) Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hire Current DEA License, or ability to obtain DEA license upon hire Previous experience with Athena EMR, preferred Strong computer skills, including Word, Excel, and PowerPoint Physical Requirements: Requires light to medium lifting, up to 50 pounds Ability to travel to various clinic sites, as needed Prolonged periods of sitting at a desk and working on a computer Must be able to access and navigate each department and clinic site Must have access to reliable transportation and ability to commute to the clinic location
