(Only QUALIFIED Healthcare Professionals accepted) Social Work - Seeking a Social Worker who provides clinical expertise to ensure all patients receive high quality, efficient care.

About the position
- Provides direct social service and counseling to patients-families
- Assists in locating appropriate resources for post-discharge care
- Participate in multidisciplinary team to coordinate the clients plan
- Cross-trains with team members to serve as back up resource
- Assesses and identifies cases for intervention
- Prioritizes goals and develops plans based on client needs
- Monitors and evaluates outcome of clients plan
- Collaborates with third party payors internal case management-disease management departments as appropriate for discharge planning needs

Requirements:
- Bachelors degree or Masters degree
- Current licensure by Texas State Board of Social Work
- LMSW, LCSW, or LPC preferred
- BLS issued by the American Heart Association
- 1 year social work experience
- acute care experience
