Aveanna Healthcare is seeking a skilled and compassionate Physical Therapist (PT/PTA) to join our dynamic team. In this role, you will provide specialized physical therapy services to patients in their homes, promoting maximum recovery, function, and health. Your dedication will aid in enhancing patient independence and overall quality of life, aligning with our commitment to providing exceptional care tailored to individual needs. Assess patient condition by physical examination, medical history, and collaboration with medical team and family members. Develop and implement rehabilitation plans that promote patients' health, maximize mobility, and enhance independence. Utilize various therapy techniques and equipment to alleviate pain and improve patients' mobility. Educate patients and family members about treatment plans, exercises, and maintaining mobility and safety at home. Document patient care and progress, maintaining accurate and confidential patient records. Adjust treatment plans as necessary in response to patient progress, new research, or clinical best practices. Proficiency in a variety of physical therapy techniques and equipment Excellent patient assessment and management skills Strong interpersonal and communication skills Ability to work independently in a home environment Knowledgeable in home health regulatory and reimbursement requirements Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Physical Therapist state license CPR/BLS certification Valid driver’s license and reliable transportation, as this position requires travel to patient homes
