Lung Nodule Coordinator 

Full- time

 

The Lung Nodule Coordinator independently manages all aspects of the Lung Nodule Clinic related to patient navigation and lung clinic appointments, in conjuction with physician(s) and administrative leadership.  The focus of this role will be to manage and follow up on lung nodules found through various avenues including but not limited to high-risk lung screenings, incidental findings in the Emergency Room and inpatient admissions. The Lung Nodule Coordinator will coordinate the next level of care for patients with lung nodules and assist patients, clinicians and program stakeholders in the collection, reporting and presentation of program outcomes.

Summary:

 

 

Under the general supervision of the unit’s director or designee the CNC assumes responsibility for direction and coordination of all functions in the unit on his/her designated shifts. In collaboration with other members of the management team, this individual is responsible for the ongoing assessment of the quality of patient care services provided in the unit or department.  

 

Responsibilities:

 

 

 

 

Serve as patient advocate from first suspicious finding to diagnosis.

  • Initiate contact with referring physician and introduce Lung Nodule program and role at time of suspicious finding. Coordinate scheduling/referring of patient into the Lung Nodule Clinic or with Pulmonary Specialist in the healthcare system.
  • Be available to patients and families throughout their care as an open, knowledgeable and empathetic contact for all care needs.
  • Respond to patient challenges and assist through resolution.

Independently manages all aspects of the Lung Nodule Clinic to include but not limited to:

  • Patient identification and navigation
  • Medical Record review and physician collaboration

Streamline care coordination and paperwork by assisting patient(s) with scheduling appointment(s) and exam preparation.

  • Engages resources in patient scheduling, clinical resource management, patient access, insurance verfciation and others as necessary to ensure appropriate patient access and follow-up
  • Supports physician in clinical care of the patient doing appointments including, but not limited to, obtaining and documentation of patient medical history, vital signs lab draws
  • Assist and support with algorithms, documents, and formalized processes for transition in commonly followed care pathways.

Support providers and assist patients in understanding their treatment options and resources available (e.g. Smoking Cessation and Pulmonary Rehabilitation).

Empower patients to remove barriers with issues of insurance, transportation, child care, financial resources in conjunction with internal and external resources (ex. Social workers, Financial Counselors)

Conduct follow-up conversations as needed with all patients and communicate concerns, changes, or social needs in patient health to appropriate MD or other appropriate care providers.

  • Assists in communicating to patient and/or provider, results of lung cancer screening, tissue sampling, etc.

Attend patient care planning conference and other meetings as necessary.

  • Ensure that appropriate patient data are available and documented at multidisciplinary meetings (MDM) held to advise on clinical care and next steps.
  • Document and complete actionable information resulting from MDM on behalf of the patient and clinician

Track and document interventions and outcomes.

  • If appropriate, support definition of datasets and ensure appropriate data are collected to track program interventions and outcomes.
  • Ensure appropriate and timely communication of patient progress to referring physicians and other care providers.
  • Collaborate with program stakeholders in support of data collection for registry participation, quality reporting, etc.   

Drive process improvement.

  • Collaborate and manage performance improvement activities as indicated by outcomes in program efficiency, patient experience and outcomes.
  • Assist in changes to the current program both locally and at a corporate level to support ongoing improvement of the program model.

Conduct outreach to referrers, providers and other medical professionals.

  • Establish and maintain positive working relationships with key internal and external customers (e.g. physicians, nurses, radiology, social services, radiation oncology, business office, shared services, etc.).
  • Educate each constituent on the role and benefits of the Lung Nodule program and high quality cancer care.
  • Recognize scope and limitations of role and regularly access clinical supervision as a support to the role.

Stay current on the growing evidence and guidelines associated with lung cancer screening and incidental nodule follow-up. 

  • Connect patient and clinician as necessary to discuss risks and benefits of screening.

Ensure service continuity.

  • Establish appropriate mechanisms to ensure service continuity during planned and/or, unplanned absence(s).

Qualifications

Education:

 

Bachelor’s Degree

Preferred

Assoiate Degree (2 yr program)

Required

Minimum (2) or more years of clinical, healthcare related experience.

  • Preferred candidates will come from backgrounds in:
  • program management or program development,
  • patient/provider relations, 
  • clinical roles in Radiology, Pulmonology, or Oncology 
  • clinical education

Required

 

License / Certification:

 

Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.

Preferred

Basic Cardiac Life Support

Preferred

 

 

Experience:

  • Minimum (2) or more years of clnical, healthcare related experience.

  • Preferred candidates will come from backgrounds in:
    • program management or program development,
